​Parents,

Athletic/Band/Flairs/Color Guard Physicals must be turned in to the Athletic Trainers in the Foster Fieldhouse. Emails will not be accepted. They will ONLY be available during the following times:
Thursday, July 31st 9-10am
Friday, August 1st 2:30-3:30pm
Tuesday, August 5th 9-10am & 4-5pm
Wednesday, August 6th 4-5pm
Thursday, August 7th 9-10am
Friday, August 8th 8:30-9:30am

All other physicals must be turned in on the first day of school. Physicals cannot be given to ANYONE other than the Athletic Trainers so that they can check them and make sure everything is completed and cleared. If anything needs to be corrected, they will let you know at that time.
The following must be present on the physical:
Parent and Student signatures
All medical history questions marked "yes" must be explained in the box on the right
Physician Signature box must have all information completed or office stamp with date of exam and signature.
